Traits

Soul Vessel. As long as his hidden vessel remains intact, Vaerûn reappears near the vessel 1d10 days after his destruction, unless the vessel is destroyed.

Legendary Resistance (3/day). If Vaerûn fails a saving throw, he can choose to succeed instead.

Terrifying Gaze. As an action, Vaerûn can force one creature he can see within 30 feet to make a DC 18 Wisdom saving throw, gaining the frightened condition for 1 minute on a failure.

Withering Touch. Weapon Attack: +11 to hit, reach 5 ft., one target. Hit: 18 (3d8+5) necrotic damage.

Flame Bolt. Spell Attack: +11 to hit, range 120 ft., one target. Hit: 33 (6d10) fire damage.

Necrotic Explosion (recharge 5-6). Each creature within 20 feet of Vaerûn must make a DC 18 Constitution saving throw, taking 44 (8d10) necrotic damage on a failure, or half as much on a success. Undead creatures are immune.

The Story

The tomb door grinds open and a green glow creeps toward you, cold as death itself. The corridors are strewn with the bones of those who came before, neatly arranged as though someone prizes them. In the distance a light pulses to the rhythm of a heartbeat that belongs to no living heart.

The seals that held Vaerûn de Grafheer in his grave for a thousand years are weakening, and with every crack his army of skeletons and spectral lords stirs deeper in the tomb. A frightened scholar knows the way in but lacks the courage to go; he presses a half-rotted map into your hands and points into the dark. The lich gathers souls, drop by drop, and he already smells yours.

You descend through a deadly dungeon: shifting traps, halls full of guardians that wake at every misstep, and riddles that open or bar the way to the crypt. Somewhere in the depths the phylactery lies hidden, the vessel in which Vaerûn keeps his soul, and as long as it stays whole, he cannot truly die.

The twist: the phylactery is not where the legends claim, and the scholar above has not told you everything. When you stand before Vaerûn de Grafheer on his throne of bones, you must destroy his phylactery while he turns the dead against you, and seal the tomb forever before his thousand-year sleep becomes an eternal waking.
